Photo forms are being handed out. Our photos will take place
Saturday October 26th - 10:30 am at the Elmira Arena. Players should
not wear equipment but should come in slacks and a collared shirt and we will have the jerseys to put over top.

Medical Information Sheet
Please review and complete all forms and return as soon as possible. The Parents Code of Conduct should be signed by
both parents
