A travel permit is required for all tournaments and exhibition games. Complete this form at LEAST 2 weeks prior to the tournament or exhibition game you are requesting.

    Tournament Details
    Complete this section only if applying for a tournament permit. See the section below for Exhibition Games.

      Tournament Start Date: This request must be made at least 2 weeks prior to the tournament start date.
    5. If you require this travel permit for a tournament OUTSIDE the OMHA (i.e. Alliance or GTHL), there is a $20 charge (per tournament). The hockey office will initially pay this fee on your behalf - drop off cash or a cheque within one month (payable to WMHA) to reimburse the hockey office.
