Please complete this form if you are a NON RESIDENT PLAYER, who will be attending tryouts in Woolwich with a NON RESIDENT PASSPORT. Please DO NOT complete this form, if you are a Woolwich Resident.

What you need to know!

Please complete this form if you are Non-Resident of Woolwich and have obtained a Non-Resident Player Passport (NRP form) from your Home Centre
Please upload a scanned copy of your Non Resident Player (NRP) passport AND bring the original to the first tryout skate (*if you have not obtained the NRP form yet, please complete the form now and email a copy of the form, once obtained, to: [email protected]. Please also bring the original form to the first tryout skate)
A Tryout Pass is $110.00 and is a one-time fee for A tryouts
