Complete this form if you want WMHA to pay for a tournament on your team's behalf. WMHA will cut a cheque for each tournament, and will subsequently bill your team for the exact amount once the season is underway. Prior to funds being "lent" to teams for Tournament fees, a postdated cheque from the Teams Bank account must be provided to Katie for the full amount - postdated to October 1st. You will be notified when the cheque is ready to be picked up in the hockey office mail slots.
