Travel Permit/Exhibition Game/Tournament Request Form

A travel permit is required for all tournaments and exhibition games. Complete this form at LEAST 2 weeks prior to the tournament or exhibition game you are requesting. Upon receiving your travel permit request, the hockey offer will submit your request to the OMHA. Please note: the OMHA approves the travel permits. The approved request will come from the OMHA, to the email you provide on this from.

If you require this travel permit for a tournament OUTSIDE the OMHA (i.e. Alliance or GTHL), there is a $20 charge (per tournament). The hockey office will initially pay this fee on your behalf - drop off cash or a cheque within one month (payable to WMHA) to reimburse the hockey office.
