In an effort to evaluate & improve our Coaches, we invite input from parents/players through this evaluation form. Please take the time to complete the mid-year survey with your son/daughter so a full perspective is captured. - This form is kept confidential, and used as a tool to identify opportunities for improvement. This form is sent to the VP Coaching, VP of Rep Teams, VP of Local League Teams and the WMHA President only. - Individual surveys will not be shared with coaches
